Aguilar OBP-3 Preamp Wiring

I've been trying to get the wiring right for the Aguilar OBP-3 Preamp to put into my MTD Kingston Artist 5.

It contains the wiring configuration for a single humbucker.

Configuration of controls is:

Volume
Tone
Active/Passive Switch
Treble
Mid
Bass
Mid Selection (400hz or 800hz)

Still not sure if it's correct, so if anything looks wrong or is wrong, please correct me!

Also note, The resistors are 1 Meg 1/4W 5% carbon resistors.

Thanks for the help to everyone, and i hope this can help people in the future once it's all correct
